Add member form should be created with the following fields

name (type: string)
email (type: string)
role (type: int)
member_type : (drop down with options FFCS or NON FFCS)
department: (dropdown: ["TECHNICAL","CONTENT","CREATIVE","OPERATIONS"])

API structure (no need to test it while development just ensure the req body is same):
