Contact form not working #486

I am not sure why but this contact form is not working https://southside.b1.church/contact In Chums I go to forms and can see it is there but when I open the page as an admin on chums I get this error. Server error when loading https://membershipapi.churchapps.org/memberpermissions/form/lk-i3opV8JH/my
