Is your feature request related to a problem or a limitation? Please describe...
limitation
In some menus it is not clear, if a function is active or not (main menu - control settings - scrollwheel for speed), maybe you could change icons correspondingly?
I looked at the icon options at: icons and see that it is not easy.

Describe the solution you'd prefer
Clear signal for the state of the options by using different icons or .... see below

Describe alternatives you've considered
You could add a check-mark next to the icon or even easier use check-boxes or open a window from menu with radio-buttons or check-boxes, whatever suits you better.

What other menu items would you change this for, besides
main menu - control settings - scrollwheel for speed, if any?

Other settings are:
main menu - control settings - throttle settings (should end in three dots, as usual when another window will pop up for the settings)
main menu - control settings - direction of arrow keys
main menu - control settings - scroll direction
main menu - control settings - scrollwheel for speed
main menu - control settings - block scrolling while prompting
main menu - other settings - always hide formatting tools (additionally the word "always" could be omitted)
main menu - other settings - autohide formatting tools
main menu - other settings - disable overlay contrast
main menu - other settings - disable background transparency
Since I only have a German version of QPrompt the terms could be a bit different in the Englich version. Maybe the hiding of the formatting tools could be better done with radio buttons?
Don't get me wrong, your program is phantastic - these are just suggestions where I feel it would be easier to use.

Maybe the items in READING REGION and INDICATORS could get radio buttons.
