There was a feature request back in the day for provider-specific credentials in this form, so this could be rewritten the same way as the provider forms. There are some necessary changes in the API and core first ManageIQ/manageiq-api#906

So far all the providers have the same form, but maybe some of them are provider specific, which I don't know. @agrare can you help me with determining which fields are common and if there are any provider specific ones? Also are all the endpoints (Default, Remote Login, Web Services, IPMI) available for all providers?

While they are all technically defined in core as options, Web Services is only implemented by Vmware::InfraManager::HostEsx

The other two (ssh/ipmi) appear to be implemented in core.
