Upgrading from 4.0.0 to 4.1.0 in MOODLE_401_STABLE

If you change fields and/or indexes during an upgrade, you should also modify your db/install.xml file.

Now I get:

remuiformat_course_visits
 * column 'user' is missing
 * Missing index 'remuiformatvisits' (unique (course, user)). 
 * Unexpected index 'mdl_remucourvisi_couuse_uix'.
 * column 'userid' is not expected (I)

This issue has been resolved in our latest release.
