I have the same problem on a fresh install of emacs on a new computer. The same init file was working on the previous computer, so maybe it has something to do with updated packages or newer emacs version?
Edit: I found the fix here in this previous issue: #88
The solution was to install the ivy-rich version on melpa as opposed to the version on melpa-stable.

@evanhackett 0.16 is very old and it's incompatible with newest ivy 0.14.0

Install package ivy-rich ver.0.1.6
