Acceptance Criteria:

Users can input the amount for either token.
User can click "max" button to populate with whole balance
Slippage tolerance field accepts input and updates state.
"Swap" button disabled when insufficient funds, incorrect amounts, etc
Output token amount updates dynamically based on price rates.
