create-pki.yml (under common-roles) is reffering the v3_ca, which is missing from the files folder:

command: "openssl x509 -passin pass:password -signkey {{siebel_base_path}}/pki/ca_private_key_file.pem -req -days 3650 -in {{siebel_base_path}}/pki/ca.csr -out {{siebel_base_path}}/pki/ca.cer -extensions v3_ca"
