Clients should have their outputs mapping updated on connect and system power-on

We should update new clients with an outputs mapping when they connect to the controller. We should also get the switcher to tell us its state after turning on (all blank, you'd hope, but it pays to ask).

At a guess, I would expect the switcher to come back to where it was last after a power cycle - that's what most pro-kit usually does.
