Please, have a look at https://tex.stackexchange.com/a/602822/5735 . I haven’t delved much into the csquotescode, but I'd say it's doing something similar to

\begin{quotation}
\begin{otherlanguage*}{hebrew}
...
\end{otherlanguage*}
\end{quotation}

which is much like \begin{quotation}\foreignlanguage{hebrew}{...}\end{quotation}. This changes the text direction, but not the paragraph one.

So I should take the fix from the linked question and simply update the csquotes code?

There are two options: (1) otherlanguage* + \bbl@dirparastext, which has the drawback it uses an internal macro, but it writes nothing to the aux file: (2) the unstarred otherlanguage, more standard, or even \selectlanguage. However, I wouldn't be surprised if there were side effects if \csq@otherlang@star is redefined, because it's used at several places.

I'm happy to test btw if that's of use. The fix in the linked question does have some odd side-effects for the citation
