Can't create deposit point (name field multilingual issues)

@asmecher I have problem, that "Create Deposit Point" does not work for me even with the update from 15 April. I have multilingual installation - English (default) and Czech - version 3.1.2.1, In apache2 error log only many warning appear, no errors - I can input data to "Deposit point" - name in both languages, URL, but I cannot change the login and password and when I click Save the button gets inactive but nothing more...

@Jirrka, I'm afraid I can't duplicate this locally... Did you apply the patch attached to this entry to an already-installed version of the plugin, or something else? Do any of the warnings look relevant?
