Multiple Yes No Questions on one page

The option is not suggested, not well tested, and not recommended. If you MUST have multiple questions display at the same time, multiple choice questions where you select one of many (radio boxes) can be shown on the same page if you change the name of the elements. By default QUEST sets the name to a default value, however you can set the name. If you do this, all the responses for a single question must have the same name. This is how html knows that a set of radio buttons go together. Here is an example of the syntax.

For the following questions assume that you are at a fine dining establishment and are about to order a salad.

Do you like onions on your salad?
(1:ONIONS) Always
(2:ONIONS) Sometimes
(3:ONIONS) Never

How about tomatoes?
(1:TOMATOES) Always
(2:TOMATOES) Sometimes
(3:TOMATOES) Never

Do you like hearts of palms on your salad?
(1:HOP) Always
(2:HOP) Sometimes
(3:HOP) Never
