docs(copm): Add sequence diagrams for COPM primitives

Primary change: adds mermaid sequence diagrams for
how COPM primitives are used in asset transfer, asset
exchange, and data sharing to copm-core README

Secondary change: adds TOC and reformats raw urls
to links
