HTML-837 Add foreignKeyTableName to liquibase constraints #298

Description of what I changed

New versions of liquibase requrire the foreignKeyTableName attribute to be set for any foreignKeyConstraintExists tags. This updates the liquibase xml. It also updates the xsd version from 1.9 to 2.0 as 1.9 does not have this attribute.
