I'm not sure what is causing the internal server errors on the build here - will merge anyway and debug issues pushing from live since we plan to remove this repo soon anyway |

Copy across the fix for the formula from rcpch/digital-growth-charts-server#209 so we can update the docs before they are merged into the main repo.
